Join us to discuss and learn more about Advance Directives, which is a written statement about a person’s wishes regarding medical treatment to ensure those wishes are carried out should the person be unable to communicate them to a doctor. Learn why it is important to choose someone who will speak for you when you cannot speak for yourself in a medical setting.

For those who want to complete the forms during our session, Trellis Support Care Staff can notarize them for you at the end.
